If you would like to apply for review of a decision under the Social Assistance Program or the Social Solidarity Program, or a claim issued by Emploi-Québec or related to specific programs, the application must be made in writing, using the Application for review form (PDF, 331 Ko). The form is also available at Services Québec offices.
When you print out the document, one copy of the Guide section (pages 1 and 2) will print out but 2 copies of the Form section (Application for review) will print out: one copy is for the Services Québec office or office that rendered the decision (pages 3 and 4) and the other copy is for the applicant (pages 5 and 6). If you prefer, you can simply choose which pages to print out by selecting them in the printing function.
To apply for review of a decision under the Québec Parental Insurance Plan,, you must complete the Application for review – Québec Parental Insurance Plan (QPIP) form (PDF, 93 KB). If you prefer, you can file your application for review by sending a letter clearly identifying the decision you are contesting. You can enclose any documents that you feel will be useful in supporting your application. If you wish, you can also request a telephone interview in order to express your point of view.
You have 90 days from the date you receive the notice of decision to file your application for review. If your application is not filed within this period, it will be rejected unless you can show that you were unable to act sooner for reasons beyond your control.
It is important to send it directly to the office that rendered the decision you are contesting, in order for the application to be processed as quickly as possible.
